FLUENZ VACCINATION RECEPTION - YEAR 6

UPDATE 5TH NOVEMBER 2020: The new deadline for completion of forms is now Sunday 15th November.

On Monday 16th November 2020, the School Immunisation Team will be coming in to give Reception - Year 6 children the nasal flu vaccine. This year they will be moving to electronic consent forms for the Flu vaccination programme.  The use of E consent is quicker and easier to use, and can be completed on any electronic device.

When following the link please ensure you select “St John’s Primary Academy-Essington” as the school not “St John’s CofE Primary Academy”.

Please ensure that you complete and submit your child’s consent form whether you would like them to be immunised or not.
